Teaching neuroimage: spindle cell oncocytoma of the pituitary gland.

A 76-year-old man had worsening generalized weakness and headache for several months. MRI showed a solid enhancing sellar mass suggestive of a pituitary macroadenoma (figure 1). Differential diagnoses included pituicytoma, paraganglioma, meningioma, and solitary fibrous tumor. Due to intraoperative bleeding, this tumor was only partially resected. Histopathology revealed interwoven fascicles of...

Pituicytoma with Significant Tumor Vascularity Mimicking Pituitary Macroadenoma

A 19-year-old man presented with bitemporal hemianopsia and was found to have a large sellar and suprasellar tumor, resembling a pituitary macroadenoma. Emergency transsphenoidal approach was attempted because of rapid visual deterioration with headache. However, the approach was complicated and stopped by uncontrolled hemorrhage from the tumor. After conventional cerebral angiography and recog...

Bilateral Tolosa-Hunt syndrome mimicking pituitary adenoma

The authors report a rare case of bilateral Tolosa-Hunt syndrome, which occurred in a 80-year-old female and remitted spontaneously. Inflammatory lesions were found not only in typical locations, i.e. superior orbital fissures and cavernous sinuses, but also in the pituitary; these imitated gland's macroadenoma in imaging studies.

Lymphocitic infundibulo-neurohypophysitis mimicking a pituitary adenoma

A rare case of infundibulo-neurohypophysitis mimicking a pituitary adenoma is presented. A 69-years-old female patient developed polyuria and polydipsia. Laboratory analysis revealed central diabetes insipidus. No hormonal abnormalities. Cranial-magnetic resonance imaging (MRI) showed a left sided mass in the adenohypophysis presuming a pituitary adenoma. The mass had contact to both internal c...

Refractory pituitary adenoma: a novel classification for pituitary tumors

Pituitary adenomas are classified as typical or atypical, invasive or noninvasive, and aggressive or nonaggressive based on pathological features, radiological findings, and clinical behavior. Only pituitary tumors with cerebrospinal and/or systemic metastasis are considered malignant carcinomas. However, some pituitary adenomas with high Ki-67 indexes exhibit aggressive behaviors, such as rapi...
